These Forms Require Physician Signatures
IMPORTANT: Forms under Step 3 must be completed for a student to attend the first day of school. Any applicable medications must be delivered to the school by the third week of August.

**If your student requires medication during the day on a regular basis, please reach out the school nurse for further instructions.**

Additional Notes:
*DO NOT send any hard copy, emailed, or faxed health forms to Liberty Christian School or staff.
*Office forms do not get uploaded into the SNAP Health Portal.
*All original copies should be easily accessible at home for the entire school year.
